Migration Review Form

This form allows holders of the historical KIYOSITO token to provide information relevant to the KIYOSITO 2.0 migration process.

Completion of this form is optional. Holders whose historical positions fall within the published migration criteria may be automatically qualified to exchange their ITO 1.0 for the new token, even if this form is not submitted.

The information submitted through this form may be used to verify the origin of historical token positions, ownership of snapshot wallets, acquisition history, participation in official distribution programs, and other matters relevant to the migration criteria.

Submission of this form does not constitute approval, eligibility, migration, allocation of tokens, ownership rights, governance rights, or entitlement to future benefits.

All submissions will be evaluated according to the published migration criteria and the supporting information provided by the applicant.
